How to fill up the Form (Step by Step Guideline)
Students must fill up the form to get an Unique ID. Without an Unique ID, Students will not be able to get an admission to the school, colleges and universities.
Students Unique ID Form is a 4 pages form. In the first two pages, there is student information, In the third pages there is students education related information and the last page there is Educational Institution related information are available.
Information Required to fill up the Unique ID Form:
First Page: Students Names (Bengali & English), Birth Certificate Number, Date of Birth, Birth District, Gender, Nationality, Religion, Class, Roll Number, Marital Status, Disability Status, Blood Group, Tribal Status, Mother Information (Name, NID, Date of Birth, Online Birth Registration Number, Mobile Number, Occupation) as Per National ID Card.
Second Page: Father Information (Name, NID, Date of Birth, Online Birth Registration Number, Mobile Number, Occupation) as per National ID Card, Present Address, Permanent Address, Guardian (In case of Father/Mothers Absence), Relations with the guardian, Guardian Mobile Number, Attestation of the Guardian, Students ID and Students UID.

Fourth Page: Co-Curricular Activities, Participation in the field of Sanskrit, Sports, Creative Work, Students Awards Information, Current Category of the students and the Attestation of Class Teacher and Head of the Students.
